HAZARD ANALYSIS

STEP SEQUENCE OF JOB PROCEDURESPOTENTIAL HAZARDS (SPECIFIC)ACTIONS TO REDUCE/ELIMINATE HAZARDS
● TRANSPORT CASING RUNNING TOOLS AND TORQUE MONITORING EQUIPMENT TO RIG FLOOR● EQUIPMENT/TOOLS COULD BE DROPPED DOWN THE RIG STAIRS WHILE CARRYING BYHAND TO THE RIG FLOOR

● EQUIPMENT COULD FALL DOWN THE V-DOOR FROM STRAPS NOT PROPERLY RATED FOR WEIGHT, WORN OUT, OR NOT PROPERLY SECURED WHILE HOISTED TO THE RIG FLOOR
● LOAD EQUIPMENT OR TOOLS INTO A BACKPACK TO TRANPSORT BY HAND UP THE RIG STAIRS

● ALWAYS MAINTAIN 3 POINTS OF CONTACT WHILE GOING UP AND DOWN RIG STAIRS

● DOUBLE CHECK THE CONDITION OF HOIST STRAPS, THEIR RATING, AND THAT THEY ARE SECURED TO EQUIPMENT BEFORE LIFTING TO RIG FLOOR.>● NEVER STAND UNDER OR IN THE LINE OF FIRE OF SUSPENDED LOADS GOING UP OR DOWN THE V-DOOR
● CONDUCT A PREJOB SAFETY MEETING TO DISCUSS EQUIPMENT RIG UP, TALLY DETAILS, AND HANDLING/PROCEDURES OF CASING RUN● REVIEW JSA WITH ALL PERSONNEL AND COMPANY REP THEN SIGN OFF ON SAFETY MEETING SHEET

● GIVE ALL PERSONNEL A CHANCE TO ADD COMMENTS, CONCERNS, OR GUIDELINES TO ALL SAFETY REQUIREMENTS AND PROCEDURES

● MAKE ADJUSTMENTS OR ADDITIONS TO JSA AT THE END OF THE TSI JSA DOCUMENT

&$9679; CONDUCT A JSA WHEN NEW RIG CREW BEGINS WORK
● RIG UP CASING CREW AND TORQUE MONITORING EQUIPMENT● EQUIPMENT TAILED IN COULD SWING INTO OTHER EQUIPMENT OR PERSONNEL● NEVER TURN YOUR BACK TO LOADS BEING TAILED IN AND STAND CLEAR UNTIL THE LOAD IS POSITIONED
● STRING ACCESSORIES BROUGHT UP TO RIG FLOOR● HANDS AND FEET COULD BE INJURED WHILE RIGGING UP EQUIPMENT

● TRIPS AND FALLS FROM POTENTIAL HAZARDS AS EQUIPMENT GETS ORGANIZED AND INTO CORRECT POSITION
● KEEP CABLES FROM EQUIPMENT ORGANIZED AND OUT OF THE WAY TO PREVENT DAMAGES OR TRIP HAZARDS

● ALWAYS WEAR GLOVES AND BE MINDFUL OF YOUR HAND AND FEET PLACEMENT WHILE WORKING

● KEEP AREA AS CLEAR AS POSSIBLE FROM TRIP HAZARDS IN THE EVENT YOU NEED TO EVACUATE THE RIG FLOOR IMMEDIATELY
● WELL CONTROL ON THE RIG FLOOR● NOT HAVING WELL CONTROL ON THE RIG FLOOR COULD BE THE DIFFERENCE IN CONTROLLING THE WELL OR NOT● BEFORE THE CASING RUN BEGINS, HAVE ALL WELL CONTROL EQUIPMENT ON THE RIG FLOOR, READY TO BE INSTALLED

● IN THE PREJOB MEETING, GO OVER THE ACTIONS AND WELL CONTROL EQUIPMENT IN THE EVENT SECURING THE WELL IS NEEDED
● RUNNIG CASING/TUBING● CONTROL OF THE INCOMING JOINT COULD BE LOST WHICH COULD SWING IN, STRIKING PERSONNEL

● YOU COULD BE CAUGHT BETWEEN EQUIPMENT OR PIPE AND THE DERRICK LEG OR OTHER EQUIPMENT IF NOT PAYING ATTENTION
● ALWAYS FACE THE WORK AND NEVER LET THE JOINT OF CASING OUT OF YOUR SIGHT WHLE JOINTS ARE TAILED IN

● MAKE SURE YOU STAND TO THE SIDE OF THE CASING AND ALWAYS HAVE AN ESCAPE ROUTE

● NEVER STAND BETWEEN EQUIPMENT AND THE DERRICK LEG OR OTHER EQUIPMENT SO YOU ARE NOT CAUGHT BETWEEN THE TWO AND INJURED

● DO NOT DISTRACT WORKING PERSONNEL ON THE RIG FLOOR
● V-DOOR OPERATORS● NOT HAVING A PERSON OR HAVING MORE THAN ONE PERSON FLAGGING IN THE OPERATORS COULD INJURE OTHERS BY CREATING CONFUSION● ONLY ONE PERSON SHOULD BE DESIGNATED AS THE FLAGGER FOR THE OPERATION AND SHOULD WEAR THE DESIGNATED VEST IF REQUIRED

● USE HAND SIGNALS TO COMMUNICATE WITH OPERATORS TO CLEARLY UNDERSTAND INSTRUCTIONS
